I don't read Japanese, but I'm pretty sure what happened here is the blonde girl clubbed the other girl over the head with that monster strap-on, knocking her senseless (presumably to steal her clothes and wallet, which are clearly not pictured.) Later, the blonde girl was haunted by her guilty conscience which appeared to her as a spectral naked guy with a P on his forehead.
