Its complicated, especially since there seems to be time jumps going around. Anyways, the "NTR" stems from the fact that her official lover is the dark-haired guy on page 2 but since we don't know when this event takes place, they may not have develop a serious relationship yet. Regardless, in effect its not Netorare since the guy doing the fucking her is the main character, making it more Netori than Netorare. Lastly, since he's the MC, he's not "cheating" on coma girl, he's just making a harem.
