I'd like to imaging trying to get out of a lot of these setups. For the hanging ones, it's actually really hard to hang yourself. If you cock you head to the side, or even turning your head, it lessens the strangulation effect which gives the person coming in plenty of time to rescue you. The reason why it worked well in the old west and death sentences with the proper setup is the slack of rope given and height that you're dropped from which will break your neck or forcefully straighten out your neck.
