Note that the English version (for the two pages that need it) is in a gallery called "Ayanami Rei 04 [MOGUDAN]", because the idiot translating it decided he didn't like the title apparently.

Also, this scan is slightly larger and, IMO, slightly better.

I think that's because it's released on C62, right after Ayanami 3(C61), before Ayanami 4(C64) was released.
Mogudan's titles have always been confusing.
There are Ayanami 1~5 (with Rei Ayanami Rei inbetween them),
then Ayanami Rei 00,
then Ayanami Dai 1 Kai ~ Dai 4 Kai (with a "Dai 3.5 Kai" and two preview books inbetween them),
yet I haven't counted about a dozen of omake books and limited books.
