From what I came across, she was involved in a nasty car wreck. The repor was sketchy as to what really happened.

All we know is she had some kind of an accident and her time of death. That's it. Saya's family and friends have not revealed anything else. Even the Japanese cosplay community is in the dark. Some speculate or add details they think sound plausible and unfortunately there are people out there who repeat it as if it were facts. Nevertheless, we do not know exactly what Saya died of.
