So assuming I'm google translating correctly, the perverted hero is disappointed that his partner (who is from a family line that supports the heroes) was male so he turns him into tentacles (as far as I can tell it's an accident while he's trying to turn his partner into a woman). As revenge his partner then turns the hero into a woman, screws him, and when they return to "normal" the hero ends up turning back into a woman because his mind had been addicted to the pleasure; the partner then uses their new bodies to "keep the hero's sexual desire in check so he doesn't do any more unnecessary things."

An actual translation/synopsis would be great, though, kind of winging it on that one.

I think the reason why the hero turns into a woman again is that he became the slave of the tentacle. So if the mage couldn't hold his human form, the hero just synchronized to be a female again.

Seems to be the case on re-reading. I imagine that is what was signified by the crotch tattoo: since the hero is a slave to his partner's tentacle form, if the partner turns into tentacles then the hero will be forced to turn into a woman. "What, then it's fine if you just don't turn into tentacles." "Actually...this might be pretty useful."

Also, I've been curious for a while but is Aji Pontarou a guy or girl? Their bishounens always look super shoujo-esque (not that I consider it a bad thing or think only women would draw men like that).

Perhaps the author of these series is a female since all of the protagonist in these stories are incredibly feminine like you said. Since bishounens usually attracts the female population has of the anime characters community (reversed harems). Or maybe they just look feminine to make transition easier and not caused the symptoms of confused boners.
