The smoke/vapor is an effect to reproduce heat ( or steam ) because they are doing sex things. :P I guess you could say this is the artist's trademark. As for the dude's look, this is the "NTR antagonist face" you get with this artist. They all kinda look like that. It's either this or the faceless dude we often get elsewhere in most NTR stories.

I think this artist is pretty good however. The girls' eyes are always really well drawn. I wish his ( or her ) stuff could get translated one day even though you can kinda tell the plot just by watching the images.

After 5 years, the husband was led to believe that he cannot produce children, after a doctor's visit. The wife suggested that she have her former lover impregnate her as she desperately wants kids. The husband very reluctantly agrees, but under the condition that he watches. During the stream, he finds out that his wife lied about her sexual past, and that she has slept with close to 30 men and was basically a slut before she got married. She also admits to having an affair with the guy making the video.

At the end, the wife is pregnant and the husband decides to ignore her past and focus on building their future. Later, it's revealed that this was all a plot of the wife's as she has long been in love with her former lover and promised to bear his children. The wife secretly took birth control pills and bribed her husband's doctor (via sex, it seems) to tell him he was infertile.
