I think Satomi is an AV idol from the beginning, and Kousuke used her to masturbate while thinking of his friend Satona, who's completely identical to her. Satona is a normal idol, and in chapter 0 she's forced to do porn. The 2nd chapter (1) has Satona doing an event as Satomi, where the audience members get to have sex with her. Kousuke realizes that it is actually Satona during this, and Satomi shows up while watching on a monitor the last page. Chapter 2 starts with what seems to be Satona and Kousuke having sex, it develops into a more love-love relationship, and while she's asking him to impregnate her, it goes to an internal insemination scene, and then to a gang-bang AV shoot. It turns out that this is Satona, and when she goes to outside of Kousuke's apartment, she sees him with Satomi on his arm. So I think that this whole chapter it was actually Satomi with Kousuke, but I'm not completely sure. 4th Chapter (3) has Satomi doing an AV while Satona joins in part way, mentioning that they're twin sisters, which I don't think Satomi knew about, but am honestly not sure.

Note: Satomi usually has a dot on the lower right of her face.
