That would be "Koakuma": a nameless midboss from Touhou Project 6, from Patchy's stage to be precise. The fandom gave her a name and made her almost a regular character in many fanfic works. According to them, she works at Patchy's library as her assistant and librarian in Scarlet Devil Mansion. She barely exists in canon, but it's almost always around whenever the fanfic deals with Patchy or her library.

Yeah that was nice.

Hell I think Musashi-dou actually made me want to fap to Koakuma too, she's not a persistent insect like she is in every other patchy doujin ever and she's actually really fucking hot in her one image.
