There's only one totally unavoidable ntr scene as far as I know and it's in the introduction, as in if you start a new game that skips the intro, it's skipped. It happens to Kotone. The other scenes you're probably referring to only happen if you let them happen, specifically if you fail to prevent someone else from having sex with them first.
